Coming home after a hospital stay

The first week is a logistics problem

Adult children often leave the hospital with a folder, a medication list, and a feeling that someone official already arranged home. Sometimes a Medicare-certified home-health agency is truly scheduled. Sometimes that sentence was a hope. The work that remains is ordinary and heavy: who is in the house overnight, how the person gets to the Tuesday appointment, whether the walker arrived, and who notices if meals stop. The hallway is crowded for a demographic reason. The Administration for Community Living 2023 Profile of Older Americans, drawing on U.S. Census Bureau data, reports 57.8 million Americans age 65 and older in 2022, or 17.3 percent of the U.S. population, up from 43.1 million in 2012. Census Bureau QuickFacts for California (2025 vintage) reports that 17.5 percent of Californians are 65 or older. Home-based care is growing as more families prefer aging in place. Preference still has to match caregiver time and a safe house.

Questions families and partners ask

Does going home mean a nurse lives in the house?

Usually no. Medicare skilled home health is a part-time clinical episode when rules are met. Daily bathing, meals, and nights are a different job. Ask the hospital which one they are ordering.

If we live in California, is IHSS the same as home health?

No. IHSS is a Medi-Cal in-home program the county runs. Medicare home health is a skilled benefit. California Home Help explains IHSS and Medi-Cal in plain language. The county still decides the case.

Can Sevah replace the home care agency or the family?
